Health Insurance Options for a Small Team (HRA, QSEHRA, ICHRA Explained)

Many small business owners assume health benefits mean one thing: a traditional group health plan, the kind larger employers offer. For a small team, that's often expensive and inflexible, and it's genuinely not the only option — a handful of alternatives exist specifically to serve smaller employers.
A traditional group plan means the employer selects a specific plan (or a couple of tiers) and every enrolled employee gets that same coverage. It works well for larger, more uniform teams, but a small business often finds the available plans, pricing, and minimum participation requirements don't fit its size or its employees' varied needs.
A QSEHRA (Qualified Small Employer Health Reimbursement Arrangement) is built specifically for businesses under 50 employees that don't offer a group plan at all. The employer sets a fixed monthly allowance, and employees use it to buy their own individual health insurance and get reimbursed, tax-free, up to that amount — no group plan to manage at all.
An ICHRA (Individual Coverage Health Reimbursement Arrangement) works similarly — employees buy their own individual coverage and get reimbursed — but without QSEHRA's size cap or contribution limits, and it allows different allowance amounts for different classes of employees (full-time versus part-time, for example), giving a growing business more flexibility as it scales.
The right choice depends mostly on team size, budget predictability, and how much the owner wants to manage plan selection directly. A QSEHRA or ICHRA generally means less administrative burden and a fixed, predictable employer cost, while a traditional group plan gives more uniform coverage but usually costs more and requires more active management.
